Collection of medical information:
To ensure your safety, and to allow us to best understand and meet your medical and surgical needs, we will be required to collect certain personal information from you including, but not limited to, demographic details, current and past medical history and to access copies of your relevant past imaging and test results. We will also require details pertaining to your Australian Medicare and private health insurance fund details.
Disclosure of medical information:
Elements of this collected information, plus records generated within the practice itself may need to be disclosed to known third parties in specific circumstances to facilitate your optimal care – this may include information sharing with such individuals or organizations as: private hospitals (concerning your admission / surgical procedure), your referring doctor, and specific medical and allied health personnel associated with your management and treatment. While such selected information sharing is implicit in your care pathway, you have the right to withdraw consent to this at any stage or to specifically limit what information is shared with others. In addition to the above, it may also be necessary to share your personal information without your consent in the setting of an emergency or under direct legal or statutory obligation.
